Finalize your child’s registration by completing the full registration form. This form is only available in French and takes about 10 minutes to complete, and you can save your progress to finish it at your own pace.
To streamline the process, please have the following documents ready:
- Your child’s immunization record;
- Your child’s birth certificate;
- Proof of residence;
- Your child’s school report card (if they are already attending a school).
Once you have completed and submitted the form, our team will contact you to confirm the registration and answer your questions.
